Shin Nippon Biomedical Laboratories Ltd. (hereinafter referred to as "this company") recognizes the importance of personal information protection in our advanced information society and strives to protect personal information based on the following policy.

 

1. Acquisition of personal information

We will acquire personal information by lawful and fair means.

 

2. Use of personal information

We will use personal information within the range of intended use indicated at the time of acquisition, to the extent necessary in the conducting of business.

When jointly using personal information with a third party or entrusting the handling of personal information to a third party, we will conduct a strict investigation of the third party, and we will conduct the appropriate guidance and supervision in order to preserve necessary confidentiality.

 

3. Provision of personal information to third parties

We will not provide personal information to third parties without obtaining consent beforehand, except as stipulated by laws and ordinances.

 

4. Management of personal information

We maintain the accuracy of personal information and manage it safely.

In order to prevent the loss, destruction, falsification and leakage of personal information, we will take appropriate information security measures against unauthorized access, computer viruses, etc.

We will not disclose personal information, such as by transmitting it to outside parties.

 

5. Disclosing, correcting, suspension of use, and deletion of personal information

We affirm that each individual has the right to request disclosure, correction, suspension of use, deletion, etc. of personal information about himself/herself, and if there is such a request, we will respond promptly without objection.

 

6. Organization and structure

We have appointed a personal information protection manager who carries out the appropriate management of personal information.

This company conducts training on the protection and proper management of personal information for officers and employees (including full-time employees, contract employees, temporary employees, part-time employees, temporary workers, etc.) and other related persons. We will handle personal information properly in daily work.

 

7. Compliance with laws and regulations

We will comply with applicable laws and other norms regarding personal information.

 

8. Creation, implementation, maintenance, improvement of our Personal Information Protection Compliance Program

In order to implement this policy, we have established the Personal Information Protection Compliance Program, which will be handled by our officers, employees (regular employees, contract employees, temporary employees, part-time employees, temporary staff, etc.) and other related personnel, in order to implement, maintain, and continually improve said program.

1. Billing method

Please fill out the request form provided (67kb file) and mail it to us to request notification, disclosure, correction, addition or deletion, suspension or termination of use, hereinafter referred to collectively as "disclosure, etc." of personal data held by this company. Please note that we can not accept requests from customers by phone, fax, e-mail, etc.

 

2. Confirmation that the requesting party is the principal or authorized representative on their behalf

In requesting disclosure, etc. of personal data, please attach the documents described in (1) or (2) below to along with the request form.

 

(1) For requests by the principal (proof of identity), one of the following:

- Copy of driver's license

- Copy of passport

- Copy of health insurance card

- Original resident certificate

 

(2) For requests by a representative (proxy confirmation), all of the following:

- Proof of identity for the principal (any one of the four items listed in (1) above)

- Proof of identity for the representative (any one of the four items listed in (1) above)

- Confirmation of proxy authority

  · For legal representatives: documents confirming the right of legal representation (one copy of the family register)

  · For any other arbitrary representatives: the principal's seal registration certificate and a letter of proxy stamped with the registered seal

 

※ We will discard proof of identity documents sent within six months of receipt.

 

3. Fees 

We will reply in writing in response to your request for disclosure of personal data, etc., so please enclose a stamp for 800 yen with in the request form (80 yen for the type 1 envelope, 420 yen for registered mail, and 300 for delivery confirmation).

 

※ If the amount is insufficient or not enclosed, we will contact you. But if payment is not made within the prescribed period, it shall be assumed that there is no request for disclosure, etc.

5. Response

Within one month of receipt of the request, we will reply in writing to the address of the principal or representative.

We will accept complaints regarding handling of personal information by telephone or letter. Please note that we can not accept requests made by other communication methods including direct visits to our company, fax, or email.
